Feasibility (preliminary or pilot) study

When the disease is rare and/or a targeted subgroup is of interest, then specific study designs for these settings may need to be considered; see Le-Rademacher et al. (2018), Gupta et al. (2011), and Mandrekar and Sargent (2009). Similarly, there is extensive work in the literature on study designs for personalized medicine in oncology, see for example Renfro and Mandrekar (2018). A first step in designing a clinical trial is to establish what is known about the disease being studied.

When study participants are blinded, they may be less likely to have biased psychological or physical responses to intervention, less likely to use adjunct intervention, less likely to drop out of the study, and more likely to adhere to the intervention. Blinding of study participants is particularly important for patient reported outcomes (e.g., pain) since knowledge of treatment assignment could affect their response.

Thus trials investigating the effects of such interventions might employ stratified randomization based on gender. For example, two randomization schedules may be utilized; one for males and another for females. Stratified randomization ensures that the number of male participants in each treatment group is similar and that the number of female participants in each treatment group is similar. First, stratification can only be utilized for known and measurable confounders. Secondly, although one can stratify on multiple variables, one has to be wary of over-stratification (i.e., too many strata for the given sample size). The sample size must be large enough to enroll several participants for each treatment from each stratum.

One should not run this study, but rather revise the research question to reverse the benefit/risk ratio. For example, in a poorly studied intervention that is not likely to lead to a minimum clinically important difference, it is too early to design the trial for such an outcome. It would be better to revise the design to better understand mechanisms, risks and benefits, or to explore other therapeutic options altogether.

In conclusion, we hope that lessons learned from the I-SPY COVID trial will have important implications for the treatment of patients with severe COVID-19 and also for future trials in critically ill patients more generally. Third, studying a variety of agents across many months in a global pandemic has also highlighted the enormous advantages of platform trials that employ a concurrent control arm able to evolve with changes in the standard of care — a relatively unique feature of I-SPY COVID. The standard of care for severe COVID has shifted dramatically over the course of the pandemic, beginning with remdesivir in late spring 2020 and the addition of dexamethasone shortly thereafter.

Note that it is very difficult to fix a poorly-designed study once it is implemented. A trial’s ability to provide the intended evidence hinges on appropriate design, from background knowledge and trial rationale to sample size and interim monitoring rules.

Through a series of lectures and case studies, this course will explore the range of paradigmatic design responses to the climate crisis. For day-to-day temperature variability, we find a clear divide between relatively certain gains for a few high-income countries and less certain, smaller losses for many lower-income countries as a result of variability increases in lower latitudes24. While extreme precipitation increases in most regions, projected damages are highest and least uncertain for middle- and high-income countries in higher latitudes such as China and the United States31. In contrast, low-income countries are more likely to face losses from shifts in precipitation deviation and wet days, but high uncertainties limit the conclusions that can be drawn.

At +3 °C, extreme precipitation reduces global GDP by 0.2% (0.1–0.4%), with 99% of our impact distribution indicating economic losses as extreme precipitation increases around the globe (Supplementary Fig. 5). This is somewhat expected because extremes have a lower temporal and spatial correlation. Therefore, aggregation from daily, location-specific events to annual indicators and country-level projections reduces signals more strongly compared to annual mean temperature13,14. However, a 0.2% GDP loss due to extreme precipitation globally for an average year still represents a tenth of the damage caused by the catastrophic 2022 floods in Pakistan, estimated at 2.2% of GDP29. Global GDP losses from extreme precipitation are compensated, on average, by temperature variability reductions in higher latitudes (+0.1% of global GDP at +3 °C)24,30 and fewer wet days (+0.2%).

While under the status quo, 54% of the global population is projected to face damage of at least 15% with a likelihood of at least 5% at +3 °C of warming, this increases to 68% of the population when variability and extremes are included. The share of the global population facing catastrophic impacts of 20% or higher with a 5% chance rises from 4% to 17%.
